Student Affiliate registration is due Sept. 23rd.


REGISTRATION INSTRUCTIONS


Get the Registration Form


  1. Fill out Registration Form:

    • Teachers with student affiliates last year.

      • A copy of your registration form from last year was mailed to you.

      • Put an X next to the names of students registering again this year.

      • List in alphabetical order any new students at the bottom of the page.

    • Teachers registering students for the first time:


      • Fill out the Student Affiliate Registration form found in your yearbook OR fill out the interactive SA form found on the TMTA website. Print off 2 copies of form before exiting out of the form.

  2. Make a copy of registration form for yourself.


  3. Mail original registration form and one check to MCMTA for $12.00 per student to: Denise Eibel.
